Step 1 - how do I lodge a claim online?
Please read the following instructions carefully as unsuccessful upload of your Online claim form may lead to a delay in the assessing process.
You should answer every question on page one and two of the form. Please provide as much detail about the injury as possible. Please read the relevant statement on page three carefully before filling in your name and the date. To receive payments promptly if the claim is accepted, injured workers should fill in their bank details on page four of the form.
Please follow these instructions to lodge:
- Download and complete the Online claim form
and ‘Save As’ to your computer. - Save the completed form as a Microsoft Word document, with the filename ending in .doc (for example, Jane Smith claim.doc).
- Close the saved document.
