Injured workers and employers can now lodge or report a workers’ compensation claim online, using our Online claim form.
Before lodging a claim online
Before lodging a claim online, the injured worker needs to have seen a doctor and obtained a Workers’
When lodging online, only one form is needed for both the injured worker and the employer. The injured worker or the employer can fill in the form - it may be helpful to fill the form in together.
It is very important for injured workers to see their doctor as soon as possible and get a Workers’

You will need the following documents to lodge a claim online:
- an Online claim form
- a Workers’
Compensation Medical Certificate - a Tax File Number Declaration (if taking time off work).
You are able to upload the Online claim form, all other documents need to be faxed or posted to WorkCover Queensland. Please include a cover sheet
stating that an application has been lodged online.

When completing the Online claim form, you will need to provide details of the worker’s wages/salary and normal weekly earnings.
