Online claim troubleshooting
You may receive the following error message when uploading your completed claim form:
Your file upload was unsuccessful.
Listed below are some potential reasons why you can not upload your claim file:
- you have tried to upload an open document:
please ensure the Online claim form is closed and try to upload the file again.
- you have tried to upload an unapproved form:
please only upload the approved Online claim form
- you have tried to upload more than one form:
you can only upload one form at a time.
Please ensure you follow these instructions to lodge your claim online:
- Download and complete the Online claim form
and ‘Save As’ to your computer. Save the completed form as a Microsoft Word document, with the filename ending in .doc (for example, Jane Smith claim.doc). - Close the saved document.
- Click on 'Step 2 - upload an online claim form’ to upload the file. Click the ‘Browse’ button, select your saved claim form and click on ‘Upload’.
- If your upload is successful, click on ‘Next’ to complete the process.
- If your upload is unsuccessful, and you receive an error message, click ‘Go Back’ to try and upload the form again. Make sure the document is saved as a Microsoft Word document (filename.doc) and that it is closed before you upload. Only the approved Online claim form can be uploaded.
