A new request for surgery 
form has been designed to streamline the surgery request process for you. This form has fewer questions, however it is mandatory for you to answer all questions. There is no longer a fee associated with the surgeon completing the request for surgery. You, as the treating surgeon will need to provide the surgical item number/s for the proposed surgery. IME provider application
To apply to become an independent medical examiner, use our online application form. The IME panel was established on 1 November 2009. All panel application received after 1 November 2009 will be considered on a quarterly basis.
Use our provider online service to review remittances and check the progress of invoice payments.

Overview of WorkCover Queensland
Put simply, WorkCover Queensland provides insurance coverage for Queensland employers, compensating and helping workers with their work-related injuries. We are the major workers' compensation insurer in Queensland. For more information about us and our processes, go to our 'Working with us' page.
Payments
We use electronic funds transfer (EFT) for provider payments.
To ensure your payments are processed quickly, straight in to your business account, call us on 1300 362 128 to provide your EFT details or complete the EFT application form and submit online. Legislatively, we are unable to pay invoices submitted more than 60 days after the date of service. More information on payments can be found in our 'Working with us' section.
