Claim lodgement

If a worker has been injured at work, they are entitled to lodge an application for compensation.

An easy way to lodge a claim for both workers and employers, is through our online claim form. It is beneficial to fill the form in together and only one form is needed for both the injured worker and employer.

Employers can provide details of an injury in the following ways:

• by using our Online claim form
• over the phone by calling 1300 362 128
• by completing an Employer's Report   and faxing it to 1300 651 387 or posting it to
  GPO Box 2459, Brisbane QLD 4001.

If a worker lodges a claim with WorkCover Queensland online or through their treating doctor, we will contact you to confirm the details of the injury over the phone.

If a worker gives you their documents, you must send them to us as soon as possible along with a completed Employer’s Report or Online claim form.
