If you have an electronic version (saved or scanned) of the claim form, you can lodge your new claim here. You can also lodge any other electronic documents relating to that claim.
I need a claim form or more information...
To lodge a claim online, you can download and complete the online claim form and lodge it below.
We can process your claim with only this claim form but supplying other documents can assist us to determine and pay the claim more quickly. These documents may include:
- a Workers’
Compensation Medical Certificate - a Tax File Number Declaration—if you are taking time off work
- details of the injured worker’s wages/salary and normal weekly earnings.
If you have electronic versions of these documents, you can lodge them below.
Otherwise you can send them to us by fax to 1300 651 387, or post to GPO Box 2459, Brisbane QLD 4001. If you are sending by fax or post, please include a cover sheet stating that the claim has been lodged online.
