Doctors can invoice us for an extra $10 (plus GST) payment when they send us the workers’ compensation medical certificate and use reference number 300078A. All documents must be received within 24 hours of the initial consultation and faxed to 1300 651 387.

You'll only receive the $10 payment when an invoice accompanies the medical certificate with completed mandatory fields (section A and F). Please write clearly on the documents you fax to us—we may reject faxes that are illegible.

You can lodge using the new medical certificate or an older medical certificate and a claim form. To receive the $10 payment, you must lodge all the documents as one single package.

New medical certificate

Send us:

  • the updated workers’ compensation medical certificate (version 3—with tick box for new claim or ongoing claim)
  • the doctor’s invoice for the initial consultation, which specifies the item number (300078A) and the $10 (plus GST) payment.

Claim form plus medical certificate

Send us:

  • the claim form completed by the injured worker
  • the workers’ compensation medical certificate (with no tick boxes)
  • the doctor’s invoice for the initial consultation, which specifies the item number (300078A) and the $10 payment.

For new medical certificates and claim form plus certificate, the doctor fax fee is payable to doctors for initial consultations only—subsequent consultations are not eligible.
