How do I lodge a claim?

Online

To lodge your claim online, complete the online claim form and follow the prompts.

Over the phone

Call us on 1300 362 128 to lodge your claim over the phone.

At your doctor's surgery

Lodge your claim at your doctor's surgery.

By fax

Fax your completed Application for Compensation form, along with your Workers’ Compensation Medical Certificate to 1300 651 387.

By mail

Send us your completed Application for Compensation form, along with your Workers’ Compensation Medical Certificate to:

GPO Box 2459

Brisbane QLD 4001.

Once we receive the claim, we will talk to you and your employer.
