Providers can check their PMP anytime in Provider Connect.

They can see its progress without contacting us. 

Save time by reusing information 

Providers can copy details from a previous plan instead of starting from scratch.

This saves time and makes it easier to prepare a new plan. 

Start using the online form from 22 September

From 22 September 2026, providers can submit PMPs using the online PMP form in Provider Connect.

Providers can prepare now by checking their Provider Connect access and confirming the right team members can submit PMPs.
