Provider management plans

A provider management plan (PMP) tells us how a worker is progressing, what support they still need, and what they’re working towards in their recovery.

The aim is to support the worker’s recovery and help them return to work as early and safely as possible. If returning to work isn’t likely, the focus should be on improving the worker’s independence and quality of life.

To request ongoing treatment or rehabilitation services, you’ll need to submit a PMP.

How can I submit a PMP?

Submit your PMP online (recommended) 

Use our online form in Provider Connect. The online PMP is the same Provider management plan you use today. The difference is that you create and submit it through Provider Connect.  

With an online PMP you can:  

• submit your PMP online  
• track your PMP
• copy a previous online PMP instead of starting from scratch  
• download a PDF copy for your records.  

Once completed, simply click Submit. Your PMP will be sent directly to us, making it the fastest and easiest way to submit your plan.

If you prefer, you can download and complete a PDF PMP (PDF, 1.59 MB).

If you don’t have a Provider Connect account, you can register for one or send the plan using our online services. 

What happens after I submit a PMP? 

After you submit your PMP, we’ll review the information you’ve provided, including the worker’s progress, treatment needs and recovery goals.

Clear and complete information helps us make timely decisions and support the worker’s recovery and return to work.

You can expect a response from us within 5 business days of submitting your PMP.

Your PMP may be:

• approved
• partially approved
• not approved.

If needed, we’ll contact you to discuss the decision and any next steps for ongoing treatment or rehabilitation services.
